Volunteer and Clearance Information

- Required clearance information is sent and collected from the district (see clearance information below)
- Board Approval of volunteers
- Steps 1-4 must be complete before volunteering
CLEARANCE INFORMATION:
1. Submit your Act 34-Criminal History Clearance
Select "Submit a new records check" and follow the instructions
*These results are usually pretty quick and will be emailed to you. Please download and send the results to me.
2. Submit your Act 151-Child Abuse History Clearance
Instructions:
1. Click on Individual Login or Create Individual Account if you have not done clearances before
2. Select Access My Clearances
3. Hit continue at bottom of the screen
4. Accept the terms and conditions and hit next
5. Hit continue
6. Select "Create Clearance Application" in the upper right
*These results will be emailed to you and you will need to login to your Compass account, Select Access My Clearances, Select continue at the bottom of the screen, There should be a link to view results at the bottom of the screen. Please download and send the results to me.
3. Submit your Act 114-FBI Fingerprint:

Instructions: Please use Service Code 1KG6ZJ when applying for Volunteer Clearances, you will then be directed to the Department of Human Services website. After you register and make an appointment for your fingerprints, you will have the option of getting your official results through the mail or electronically. When you receive your results, please bring them to the Central Administration Office and we will make copies for you.
4. Submit your PDE 6004 Form:
(This form requires you to notify the district if you have been arrested or are arrested for a violation of the Child Protective Services Law. It also requires you to notify the district if you have been arrested for a felony offense under Pennsylvania's Controlled Substance, Drug, Device, and Cosmetic Act within the preceding 5 years.): CLICK HERE TO FILL OUT FORM
These items should be submitted to Jody Brockett (jody.brockett@burrell.k12.pa.us) in the Business Office for Board approval. Once approved, you will be notified and added to our volunteer list. It is your responsibility to renew and submit your clearances every 5 years to remain a volunteer.
Current Volunteer:
It is your responsibility to renew and submit your clearance requirements every 5 years under the new law. Even though, a new PDE 6004 Form is not required. You are still required to notify the district if you have been arrested or are arrested for a violation of the Child Protective Services Law or a felony offense under Pennsylvania's Controlled Substance, Drug, Device, and Cosmetic Act within the preceding 5 years.
If you are already a board approved volunteer and your clearances are more than 1 year old expired, you will need to complete all the steps as if you are a new volunteer. If you are just renewing your clearances after the 5 years, follow the clearance information to the left.
If you already have an Act 114-FBI Fingerprint Clearance on file, you will submit an affidavit stating that you have been a continuous resident of Pennsylvania for the past 10 years and have not been disqualified for volunteer service due to a conviction for any crime listed in the Child Protective Services Law.
